Why he practises this way

Dr. Goh spent most of his career in high-volume, insurance-driven dentistry, looking at teeth instead of people, and it burned him out. He was also a dental-phobic patient himself, with a bad gag reflex, so he knows the chair from both sides. The training he chose afterwards points in one direction: how the mouth sits inside the rest of your health.

What actually happens

A first visit, step by step

  1. We start with your history

    The first visit is a conversation before it is an exam. Medications, sleep, family history and what your last dentist told you all change how we read what we find. Dr. Goh is not in a rush here, and this part is not a formality.

  2. The gum exam is a disease exam

    We measure the depth of the pocket around each tooth and note where the gums bleed when we probe them. Those two measures track with gum disease better than anything you can see in a mirror, and neither one hurts. That is what makes the early stage so easy to miss.

  3. One rinse, sent to a lab

    You swish and spit, and the sample goes to a lab. The panel targets eleven of the several hundred bacterial types in your mouth, and it sorts them into high, moderate and low risk. A second reading looks at one of your own genes rather than at the bacteria.

  4. We screen how you breathe at night

    Snoring, waking up tired, morning headaches, a dry mouth and a scalloped tongue form a pattern we look for. Dr. Goh screens for it, and he may suggest a sleep screening beyond the visual exam. That screening looks at sleep quality, heart rate and oxygen saturation. A sleep physician makes the diagnosis, not us.

  5. What we put back

    Every restoration here is tooth-colored composite or ceramic, and this practice has never placed an amalgam filling. Metal-free is simply how Dr. Goh has always worked, so it is not an upgrade you have to ask for. If you have a metal sensitivity, tell us at the first visit and we will plan around it.

The part that gets skipped

Why two people with the same bacteria end up differently

The bacteria are only half the story. What decides how much damage they do is your own immune response, which Dr. Goh calls the most important part of the whole picture. Some people answer a low-grade gum infection with a large amount of inflammation, and that inflammation is what dissolves the bone around a tooth.

That tendency is partly genetic, which is why the lab reads your interleukin-6 genotype alongside the bacteria. It is read once in a lifetime, because your genes do not change. We re-test the bacteria after treatment, so you can see whether what we did actually moved anything.

Researchers have tied several of these gum bacteria to conditions elsewhere in the body, including cardiovascular disease and Alzheimer's. Association is not the same as cause, and treating your gums is not a treatment for anything beyond them.

The honest answer is that researchers have found strong associations, not proof of cause. Several of the bacteria on this panel turn up in research on cardiovascular disease and on Alzheimer's, which is why they are worth naming. What we treat is your gum infection and the inflammation that comes with it. Any claim beyond that is further than the evidence goes, and further than Dr. Goh will go with you.
